Required Medical Health Forms
In preparation for the upcoming school year, all new students must have a valid physical exam, immunization record, and TB risk assessment form on file before starting classes in August.
In order for the physical exam to be valid for the entire 26-27 school year, it must be dated after May 27, 2026. The physical form, immunization record, and TB risk assessment must be completed and uploaded to your PowerSchool account by Wednesday, August 5.
Instructions for completing these forms
- Schedule your Doctor Physical appointment for any date after May 27 and before August 5.
- Use your login for the parent dashboard to access your PowerSchool account.
- Download the physical exam form, Immunization Record Form, and a TB Risk Assessment form from your student's enrollment forms in PowerSchool. Please note that these forms won't be available to download until May 28.
- Bring the blank form(s) to your appointment with your physician.
- Upload the completed form(s) to your student's registration forms in PowerSchool.
Resources for scheduling a Doctor Physical Exam
We recognize that some families can only complete their physical exam once a year through their physician. If you have already scheduled or completed your student's physical exam and are encountering issues scheduling an additional appointment after May 27, we have some additional resources available:

Athletic Physical Clearance
Incoming students who are interested in participating in Athletics will also need to upload their Doctor Physical Exam form to HomeCampus, the Athletic Clearance system used by our Athletic Office. In order to avoid having this physical exam expire during the middle of the sports season, all physicals must be completed after May 27, 2026. New families will not be able to access HomeCampus for the 26-27 school year until May 28.
