Next Steps

Power School Registration Forms and Required Medical Health Forms

 

In preparation for the upcoming school year, all incoming families must complete registration forms in our database, PowerSchool. These forms ensure that we have updated family contact information and student health information heading into the 2026-27 school year. We also use these forms to collect sizing for student health & fitness clothing and the class of 2030 rally jersey.  

  1. All families can now access their health forms in PowerSchool! Please see the PowerSchool page  for detailed instruction on how to complete these forms. These forms must be submitted by Wednesday, May 27. 
  2. All incoming students must submit three required medical health forms (physical exam, immunization record, and Tb risk assessment form)  by Wednesday, August 5. These forms must be dated AFTER May 27, 2026 so that they are valid for the entire school year. Parents and Guardians will be able to download these forms from PowerSchool on May 28.
