If your student is leaving school early due to a school-related reason (athletic dismissal, school activity), do not complete this form.  We already have a record of the early dismissal!

If your student(s) will be leaving school before the end of their academic school day for medical or personal reasons, please complete the following questions before 7:45am on the day of early dismissal and instruct your student to come pick up their campus departure pass from the Attendance Office before going to their first class.
